Self-exclusion questionnaire
By filling in the Self-Exclusion Questionnaire you will exclude the blood you donated to us from being used for the patient. It could put him at risk.
This questionnaire must be filled in immediately in the following situations:
- After donation, your health status will change. The highest risk is 72 hours after collection. If you have had a high temperature, acute diarrhea or other clinical signs of illness 72 hours after collection, please report it to us! However, we also need to know if you are diagnosed with hepatitis, HIV, Lyme disease, cancer, etc., several months after collection.
- After the donation you realizethat you forgot to tell us some facts that may affect the safety of your blood. For example, risky sexual behaviour (sex with a prostitute, sex for money, sex between men, etc.), return from a risky area, medication taken, close contact with a sick person, recent tattoos, etc.
You can also call or email us instead of filling it out, but filling in the electronic Self-Exclusion Questionnaire is the fastest way to resolve it because the information gets to the appropriate places immediately (even on non-working days) and that's desirable because time is important.
Later adverse reactions of the blood donor after donation
Drawing blood or blood components may cause an adverse reaction that will occur hours to days after your blood draw.
